Description of the job
Responsibilities:
- Assisting participants to perform important daily activities and home based tasks.
- Provide short-term focused interventions, psycho-education, and mentoring.
- Facilitating daily personal hygiene and care tasks.
- Assisting with coordinating and facilitating community access.
- Providing emotional support and encouragement.
- Facilitating participant communication and enhancing their choice and decision-making processes.
- Implementing and monitoring Behaviour Management Strategies as required.
- Goal Planning and tracking
- Empowering participants with learning, skill development and employment.
- Transporting participants to medical and other appointments, and social activities as required
- Upholding the rights, independence and dignity of all participants, ensuring confidentiality and their choices are recognised at all times.
Skills Required:
- Certificate III or IV or above in Disability or Aged Care
- NDIS Worker Screening Check
- 100 points of ID (Citizenship/Passport/Driver License/Visa & Granted Form, Medicare Card or government Issued Document)
- Police Clearance
- First Aid + CPR
- Manual Handling
- Medication certificate
- Car Rego
- Car Insurance Comprehensive
- Covid-19 Vaccinations 3 dose
